To have accountable local representatives with power is the only way of making sure that professionals work to acceptable policies and that services are focused on public needs and adequately resourced.

This way there will be no division of responsibility, either over complex organisational barriers (like the internal market) or because an external body has provided insufficient resources to achieve published policies and service standards.
